Documents necessary for registration of any new student are as follows:

1. Birth or Baptismal Certificate
2. Previous report card (if available)
3. Transfer card from previous school district
4. Three proofs of residency
5. One document should relate to property itself (deed or settlement/mortgage/ lease papers)
If property documents are unavailable, additional residency form(s) may be required to be completed and notarized until additional residency documentation is provided to the registrar.
6. Immunization Records
7. Physical (as indicated on forms)
8. TB test - Any student moving from out of state must provide proof of this test.
9. CST Records - IEP reports, Observations, etc., (for students eligible for special services.

Please Note:
For those entering grades 5 through 8, last year's report cards and standardized test results are required for placement.

Kindergarten Registration Requirements:

All children who plan to enter kindergarten next September must meet the following requirements:

1. The child must be 5 years old on or before October 1, 2007.

2. The child’s birth date must be documented by an original birth, or baptismal/religious certificate. One of these certificates must be presented at registration.

3. A physical examination is required for all kindergarten students. This should be completed as soon as possible.

4. Three proofs of residency must be presented at the time of registration, one of which should be a copy of a lease agreement, deed or agreement of sale.

5. In accordance with the New Jersey Department of Health, each child must be completely immunized before entering school against the following:

Diphtheria, Tetanus and Pertussis (DPT) Vaccine:
Children less than 7 years of age: minimum of 4 doses are required. One dose must have been administered on or after the 4th birthday. Alternatively, a child with any total of 5 doses will be in compliance.

Polio Virus Vaccine:
Children less than 7 years of age: a minimum of 3 doses of polio virus vaccine (OPV or IPV) provided a least 1 dose is given on or after the 4th birthday. Alternatively, a child with any 4 doses spaced by a minimum of 1 month will be in compliance.

Measles/Mumps/Rubella (MMR);
Two doses of MMR given after the first birthday will be required of all children born on or after January 1, 1990. The two doses of MMR vaccine must be separated by an interval of at least one month.

Varicella (Chicken Pox):
Beginning September 2004, one dose of varicella vaccine on or after the first birthday OR proof of disease immunity will be required of pupils entering kindergarten or grade 1, born on or after January 1, 1998.

Hepatitis B Vaccine:
*Children born on or after January 1, 1996 (attending or transferring into a New Jersey school) are required to have 3 doses of Hepatitis B vaccine.
*Children born on or after January 1, 1990 (attending or transferring into a New Jersey school) are required to receive 3 doses of Hepatitis B vaccine.
Children presenting documented laboratory evidence of hepatitis B immunity or a physician’s written certification of past hepatitis B disease constitutes a medical exemption and shall not be required to receive hepatitis B vaccine.

Tuberculin Testing
Mantoux intradermal tuberculin skin testing is required for those students who transfer into Mount Laurel Township Schools if:
*Transferring from another state (if required) EXCEPT
- K-5 grade transfers with documented Mantoux skin test result at age 4 or after;
- 6-12 grade transfers with documented Mantoux skin test result within the past 6 months.
*Transferring from another country without a documented Mantoux TB skin test within the previous 6 months.
