Mt. Laurel Township School District is participating in the Special Education Medicaid Initiative (SEMI) program that allows school districts to bill Medicaid for services that are provided to students.

Pre-K to Grade 4 Physical Form if you are updating an existing student's physical information. Must be completed by physician and parent.
 

If your child plans to participate in an interscholastic/intramural sport or club, the Medical Eligibility Form contained in the packet above, must be completed by your child's health care provider AND returned to the Nursing Office.
